(לסי' קכט). סיום סי' קצא. החזיק עצמו יחזקאל בן מרדכי יעקב ואומר ששמו משה בן יעקב ומחמת סיבה שינה שמו ושם אביו. לכתוב יחזקאל בן יעקב וכל שום כו' אבל צריך שאלה מהבעל שלא יקפיד בכתיבת וכ"ש כדי שלא יוכל לקלקלה.
