בדין שכ"מ ששינו את שמו שהיה שמו נפתלי וקראוהו שמאי וגירש ביום השינוי. וכ' בגט שם השינוי ועל שמו הראשון כתבו דמתקרי היינו שמאי דמתקרי נפתלי. ואחר עשרה ימים מת וחיים לכל ישראל שבק אם הגט כשר. כי מת בלא בנים ויש לו אח קטן בן ששה שנים:
