נושא1 תעודה זו, היהודי הרב זלמן בן ברוך, אשר מצוי כאן בענין מסויים, עכשיו [כשאין] בו צורך, הוא משוחרר לביתו שנמצא בפלך בילורוסיה. לשם כך, כדי שעם הגיעו לוויטבסק יתייצב עם תעודה זו לפני שר-הפלך האזורי של בילורוסיה, האדון יועץ הסתרים ז'גולין.
